Care Management: The Absent Patient

Perhaps the most challenging type of patient is the one who does not come in for visits routinely, limiting the opportunity to understand and address the absent patient’s needs and barriers to presenting for care. Similar to poorly controlled patients, population management can assist diabetes health care teams in identifying and notifying patients who do not routinely schedule or attend visits. Standardizing approaches to issuing patient notifications to this subset of the patient population can increase efficiency and minimize required resources.
