Updated NDEP resource helps improve delivery of diabetes care
To help health professionals better care for people with diabetes, the National Diabetes Education Program (NDEP) has updated its resource, “Practice Transformation for Physicians and Health Care Teams,” designed to help medical professionals change systems of health care delivery around diabetes.
Formerly known as “Better Diabetes Care,” this free online resource provides models, links, and tools to help physicians and health care teams make long-term improvements that are essential to providing evidence-based care for diabetes and its complications. Improvements can also help transform a practice into a Patient-Centered Medical Home (PCMH). Content is based on current, peer-reviewed literature and evidence-based clinical practice recommendations.
“Practice Transformation for Physicians and Health Care Teams” has sections dedicated to engaging leadership and assessing your practice, providing evidence-based care, using information systems, improving practice quality, using clinical decision support, practicing team-based care, enhancing patient interactions and improving patient care coordination.
