National Diabetes Education Program enhances resources for health care professionals

NDEP has updated its popular GAME PLAN for Preventing Type 2 Diabetes: A Toolkit for Health Care Professionals and Teams. This online toolkit helps health care professionals identify, counsel, and support patients at risk for type 2 diabetes. It includes prediabetes screening information, strategies for talking to patients about prediabetes, and multiple tools and resources to help patients make lifestyle changes to prevent or delay the onset of type 2 diabetes.

To support health care professionals in improving medication-taking behavior, the NDEP has created the Promoting Medication Adherence in Diabetes web resource. Featuring handouts and resources that health care professionals can use with their patients, this site also includes videos, presentations, training guides, assessment tools, as well as jounal articles related to medication adherence.
